Consent to Participate and Waiver: I understand that participation in the activity of attending class or performances (hereafter the "activity') involves a certain degree of risk. I have carefully considered the risk involved and have given consent for myself and/or my child to participate in the activity. I understand that participation in the activity is entirely voluntary and requires participants to abide by applicable rules and standards of conduct. I voluntarily assume all risks of loss, damage, or injury, including death, that may be sustained by my and/or my child's involvement in the activity, as well as all risks occasioned by use of the Phoenix Irish Arts of Atlanta and/or any facility used by the Phoenix Irish Arts of Atlanta.
In consideration for permission to participate and other good and valuable consideration hereby acknowledged: i (we) waive my right to bring an action against Phoenix Irish Arts of Atlanta, its agents and employees. I (we) release Phoenix Irish Arts of Atlanta and all its employees, volunteers, related parties, or other organizations associated with the activity from any and all claims or liability arising out of this participation.
This release and covenant not to sue shall be binding upon the distributees, heirs, next of kin, executors and administrators of each of the undersigned.
Medical Release: In case of an emergency involving my child, I understand every effort will be made to contact me. In the event I cannot be reached, I hereby give my permission to the medical provider selected by the adult leader in charge to secure proper treatment, including hospitalization, anesthesia, surgery, or injections of medication for my child. Medical providers are authorized to disclose to the adult in charge examination findings, test results, and treatment provided for purposes of: (a) medical evaluation of my child, (b) follow-up and communication with me and/or my child's other parent or guardian, and/or (c) determination of my child's ability to continue in the program activities.
Photo & Video: I hereby authorize Phoenix Irish Arts of Atlanta, which houses the Phoenix School of Irish Arts and IrishFest Atlanta, among other projects, to use photos taken of me participating in events related to Phoenix Irish Arts of Atlanta in publications and media related to Phoenix Irish Arts of Atlanta. Such publications are not limited to, but may include, websites, digital and physical posters, advertisements, public articles and other such media.
I acknowledge that since my participation in the above publications and media produced by Phoenix Irish Arts of Atlanta and its projects, I will receive no financial compensation.
I further acknowledge that my participation in any such publication or media produced by Phoenix Irish Arts of Atlanta and its projects offers me no rights of ownership whatsoever. I release Phoenix Irish Arts of Atlanta and its projects, contractors, employees and volunteers from liability for any claims by me or any third party in connection with my participation.
COVID SAFETY POLICY: No one with symptoms of COVID 19 including Fever, Headache or Cough or known close contact to someone with COVID 19 shall enter the premises. Staff and teachers are responsible for asking all students and individuals entering the premises if they are experiencing Fever, Headache or Cough or have had known close contact with someone with COVID 19.
In the event that PSIA needs to cancel in-person classes for music or dance due to COVID developments beyond PSIA control, every effort will be made to hold classes online. There will be no “refunds” issued for in-person classes canceled when a replacement online class is made available.
